Medical Forms

All Scouts and adult leaders must have Parts A & C of the medical form on file with the Troop.  In addition, Scouts and adult leaders participating in summer camp or in any high-adventure activities are required to have Part B of the form on file.  Please direct any questions regarding medical forms to the Scoutmaster.
